Please make sure that you have chosen the correct location (Dallas, Plano or WinStar) prior to booking your appointment.
A credit card is required in order to book and hold your appointment. Your card WILL NOT be charged upon completion of the online reservation, but your card MAY be charged in the case of a no-show or a cancellation within 24 hours of the appointment time.
